According to a theory on Reddit, it seems that Doctor Doom is one such variant from a different timeline who couldn’t cure the palladium poisoning. To keep himself alive, he stayed inside the suit until he could no longer live without it.
Going crazy in his suit and going to Latveria for a cure, Doctor Doom is born with a Tony Stark who has gone crazy in search of a cure.
