There are lots of quest items in Mewgenics that drop from strange sources, do unusual things, and normally need to be dealt with via some mysterious circumstances. It’s a game of trial and error ,where the trial and error can often get you killed. If you’ve found the Putrid Leech, or are wondering where to get it, then be glad to know that I have failed as many times as it took to find out what you need to do.

To get the Putrid Leech quest item, you will need to play through the game until Guillotina attacks you for the second time.  After this fight, you will receive the Putrid Leech, which, once again, is a cursed item. You will not level up while you are carrying this item, and you will die when you are downed. The good news is that your abilities and attacks will have lifesteal, which will help keep you alive. 

Guillotina is a slightly different fight this time, but I don’t want to spoil it, so just prepare for a good scrap. Remember, you can only take her on with retired cats, so level up some strong options on a run and keep them around for when she shows up.

To use the Putrid Leech, you need to bring it to the Throbbing Artery, which can be found in the Boneyard. The Boneyard is unlocked by making your way through the Alley, finishing the Alley on Hardmode, then going to the Junkyard, and finally the Boneyard.
